Voice note: No, it's definitely not. It's summer. Bloody hell, I'm boiling. I drive soon, but my dad is getting discharged. They're just waiting for the medicine and the letter. But sometimes that takes 3 or 4 hours, but this started it a while ago. Might be in one hour, might be in two, I don't know. So I'm going to drive to you for now. And if they phone me, it's time to go, then I'll go and pick him up.

The overall picture is: your father had a significant anterior heart attack caused by a blockage in the LAD, that artery was successfully opened and stented, but he still has significant disease in two other coronary arteries. His heart pumping function has been moderately reduced by the heart attack, to around 40–45%, but the doctors expect there is a reasonable chance it will improve with medication and recovery. They are deliberately not stenting the other arteries at present unless they cause symptoms. What happened to his heart The doctor confirms that several coronary arteries have disease/blockages, but they identified one particular artery as the culprit for the heart attack: the LAD — left anterior descending artery. That fits extremely well with what happened when your dad was brought in: his ECG showed an anterior STEMI, meaning the front part of the heart was being deprived of blood. The LAD supplies that territory. During the angiogram they injected contrast dye. They could see the dye either stop or become very restricted at the blockage. They put a stent across that area and then injected contrast again, showing blood flowing through properly. That's how they know that the procedure successfully reopened the culprit artery. So, importantly, the stent worked. But he still has two other diseased arteries This is probably the most important new information. Your father appears to have multivessel coronary artery disease. The LAD was treated, but the doctor confirms that two other coronary arteries remain significantly diseased/blocked and untreated. AUDIO-2026-09-14-15-15-46.txtTXT That doesn't mean the doctors have forgotten about them. The doctor explains that there are risks associated with putting in additional stents, so they don't automatically stent every narrowing they find. Their current approach is essentially: Treat the culprit LAD → give strong preventative/heart-protective medication → see how he recovers → intervene on the other arteries if they actually cause problems. If he develops exertional angina — chest pain when walking or exercising — the remaining arteries may need reconsideration. If he develops severe chest pain at rest like he did with this heart attack, he should return to hospital immediately. Conversely, if he feels completely well, the doctor says he could potentially live indefinitely with those remaining blockages without needing another procedure. That sounds alarming when described as "two blocked arteries left untreated", but medically it isn't necessarily inappropriate. Whether additional PCI/stents are beneficial depends on things such as how severe and physiologically important the stenoses are, their location, symptoms and overall anatomy. The heart attack has damaged his heart — but not catastrophically This is the other major finding. His echocardiogram shows an ejection fraction (EF) of approximately 40–45%. EF is basically the proportion of blood the left ventricle ejects with each contraction. The doctor's rough explanation was: - Normal: roughly 55–65% - Your dad: 40–45% - Much more severe impairment can be around 30% or below So his heart isn't pumping as strongly as before the heart attack, but this isn't the sort of extremely poor function sometimes seen after a devastating MI. The doctor calls his impairment roughly mild-to-moderate/moderate, and specifically says: "It's not terrible. It's a bit damaged." More importantly, the doctor thinks it may improve. Why he's suddenly taking so many drugs The doctor specifically mentions: Ramipril + bisoprolol + dapagliflozin, alongside his other post-heart-attack medicines. Those three are particularly important because they reduce the workload on the damaged heart and help prevent adverse cardiac remodelling. Over the following weeks/months, the stunned/damaged myocardium can recover to some extent. The doctor explicitly tells him that these medicines are intended to help the heart "remodel and restore function" and says the likelihood is that his pumping function will improve, although nobody can promise how much. The transcript doesn't reliably give the entire medication list, so I wouldn't infer the others from this recording alone. What they want you to watch for Because his EF is reduced, the doctor explains symptoms of heart failure that should trigger attention: Increasing ankle/leg swelling; worsening breathlessness; breathlessness when lying flat; or waking at night unable to breathe and improving after sitting upright. Those can indicate fluid accumulation because the heart isn't pumping effectively enough. Mild swelling → contact GP/clinical team. Severe breathlessness/inability to breathe → urgent medical attention. That doesn't mean the doctor thinks he currently has severe heart failure. They're explaining what to look out for because his EF is reduced following the MI. His lower blood pressure is expected Someone raises the fact that his BP has become considerably lower. The doctor's answer is reassuring: this is probably a consequence of the new heart medications and lower BP is acceptable provided your dad feels well. The important distinction is symptoms. If he's walking around normally without dizziness, fainting or significant unsteadiness, they're willing to tolerate a somewhat lower blood pressure. If he's getting dizzy when standing, fainting, or becoming very wobbly, the medications may be lowering his BP too much and the GP/cardiology team needs to know. The doctor also agrees that monitoring his BP at home would be sensible. Follow-up There are actually several layers planned. The doctor says: ~6 weeks: telephone follow-up. ~6 months: cardiology review, apparently involving the team/consultants responsible for his angiogram. Earlier assessment if symptoms recur. They're also arranging for the cardiac rehabilitation team to see him before discharge, because apparently this hadn't happened yet. Cardiac rehab is quite important here. It should give him a structured programme for gradually increasing walking/exercise rather than simply leaving him to decide how much activity is safe. Do they need another scan to check the stent? Probably not routinely. The doctor explains that once they know the stent has opened the artery, they generally don't repeatedly scan it simply to make sure it's still open. If he takes his medication and remains symptom-free, they leave it alone. If significant chest pain or other concerning symptoms return, that's when they reconsider angiography or other investigations. Flying and driving Your dad asks specifically whether he can fly. The doctor says: Four weeks no driving, four weeks no flying. After four weeks, provided he feels well, the doctor says flying should be okay and advises making sure he has appropriate insurance. I would treat that as this doctor's advice for your father's particular case, rather than a universal rule for everybody after an MI. The part I would want clarified before discharge There's one thing I'd particularly want nailed down. We now know that this wasn't simply "one blocked artery, stent fitted, problem solved." He has multivessel coronary disease, with the LAD treated but two other diseased arteries remaining. AUDIO-2026-09-14-15-15-46.txtTXT I'd want the discharge paperwork to tell us exactly which other coronary arteries are affected and the percentage/severity of each stenosis. For example, there's a substantial difference between a moderate 50–60% narrowing and a critical 90% lesion. I'd also want to know whether the remaining lesions were assessed physiologically (for example with FFR/iFR) and whether the cardiology team's plan is explicitly medical management unless symptomatic, or whether staged PCI is still being considered. That's particularly relevant given what happened previously: your dad had repeated severe post-meal chest pain, was initially thought possibly to have a gastrointestinal cause, was discharged, and then returned with the anterior STEMI. I therefore wouldn't want future recurrent pain automatically dismissed simply because it occurs after eating. Where I think your dad stands now This meeting is more reassuring than frightening, although he's certainly had a serious event. His major LAD blockage has been successfully treated. His heart has taken measurable damage, but an EF of 40–45% is nowhere near the worst levels the doctor sees, and there's a realistic prospect of improvement as the heart recovers and the medications take effect. The remaining coronary disease needs surveillance, but the cardiology team doesn't currently think the risks/benefits justify automatically stenting everything. The next 6–12 weeks will tell us considerably more about where his heart function settles.

Voice note: called me from Virgin Media and sorted everything. And plus he's, I don't know, he dropped the price a few pounds and he's making the mobile phone. It's going to be unlimited as well. And they'll come and install it Friday morning. So that's sorted. And let's hope that the sofa comes on Friday as well. And everything's sorted. But we'll see.
